Back in February 2008, during my final professional examination for semester 7, I encounter this one obstetrics station during my Objectives Structured Practical Examination (OSPE) paper. I have been given a thing, looks like a transparent plastic straw, with some curly thing inside it. The length is about 30cm. At one end of the straw, I can see that it is thin and sharp. The plastic straw is not hard but not soft either. Suffice to say that it is fairly flexible.
Based on my limited experience, I know that I do not know what that thing is. It looks like an amniotic hook for me but I know that it is not. During my obstetrics posting, I have been assigned to the labor room for a week, but I never encounter any doctors or medical staffs using it (or may be they used it when I was not there).
It is only a five minutes station and I have not so much time and choice. Not that I want, but I need to write “amniotic hook for artificial ruptured of membrane” as my answer for the question of “What is the name of the device given?” For me, that is better rather than leave my answering paper blank. Worst come to worst, maybe I can get some mark on my “effort” to answer that question even though I do not know the correct answer.

I know that I am not alone that time. More than half of my friends said that they answered the same thing as we are not sure exactly what it is. I came to know that the “thing” is actually a fetal scalp electrode. DAMN!
What is fetal scalp electrode?
Fetal scalp electrode actually is just another device that plays a role in fetal surveillance and fetal monitoring. The function is just like an external cardiotopograph, which is for recording the fetal heart rate and accessing the fetal condition. The different is fetal scalp electrode need to be inserted in to the mother’s uterus and do you still remember the thin and sharp end that I have mentioned earlier? That one must be touched on the fetal skin or more precise, the fetal scalp (assuming that the normal presentation of the fetus during labor is cephalic presentation).
What is the indication of the using of the fetal scalp electrode?
Until today, I am still wondering. Based on my reading, fetal scalp electrode is used whenever the obstetricians or the doctors in charge want to have a more accurate reading of the fetal heart rate. In the other words, fetal scalp electrode is used as internal fetal heart monitoring when the external fetal heart monitoring is insufficient or a closer surveillance is needed. I still do not know the exact indication.
Is there any contraindication of the using of the fetal scalp electrode?
From my limited reading, there are two main conditions where fetal scalp electrode is contraindicated.
1) When the mother is having a serious or active infection on the genitalia such as herpes. The logic is we do not want to convey the infection to the fetus.
2) When the mother is having an evidence of polyhydromnias. The reason is because, for the usage of fetal scalp electrode, we need to do an artificial rupture of membrane (if the membrane is still intact). In the case of polyhydromnias, there will be an increased in the risk of cord prolapse. If this happen, it will put the fetus in an unwanted danger condition.
What is the risk of the using of the fetal scalp electrode?
Since we will introduce an outside material to the inner side of the mother, the risk of infection is very true. Other than that, due to the sharp end of the electrode it may cause bruises on the fetal scalp or the other body parts.
That is a few things that I learn and know about fetal scalp electrode. However, above all, the most important thing that we must know is how to differentiate between fetal scalp electrode and amniotic hook. So here I provide you the pictures of the amniotic hook for the comparison.
Do not repeat the mistake that I did.
